TH6.5 Perioperative group and save testing are not routinely indicated for emergency laparoscopic appendicectomy and laparoscopic hernia repairs

Abstract Introduction Major haemorrhage is a rare complication of laparoscopic hernia repair and appendicectomy. It common practice for patients undergoing these procedures to have 2 valid group saves preoperatively, however perioperative blood transfusions seldom required. Obtaining samples not only financially burdensome but frequently leads delays in theatre lists patient care. Our unit therefore performed retrospective cohort analysis investigate perioperatively within 28 days appendicectomy repairs. Method We used our electronic records system collect data all appendectomies repairs between March 2017-March 2021. Patients any age or were included. requiring concomitant intra-abdominal surgery had incomplete medical excluded. Results 1893 included the study which 1464(77.3%) vs 429(22.7%) repair. In total 3511 taken costing £47,398.50. Only 1 (0.068%) required an emergency transfusion (4 units red cells) secondary major (0.11%) pre-operative anaemia. Conclusion findings demonstrate that incidence low, challenging indication routine preoperative saves. Considering findings, burden cost, discomfort delays; we advocate more nuanced approach saves, reserving stratified as high risk.
